1. INTRODUCTION
Welcome to 4M Institute LLC (“4M Institute,” “we,” “us,” or “our”). We operate a dental education and training institute providing continuing education courses, workshops, mentorship programs, and related educational services for dental professionals and students. These Terms of Use (“Terms”) govern your access to and use of the 4M Institute website, registration platforms, and all related pages (collectively, the “Website”), as well as your participation in any Institute programs. By accessing or using the Website or registering for a course, you agree to these Terms. If you do not agree, do not use the Website or register for Institute programs.
2. ELIGIBILITY
Courses are intended for licensed dental professionals and, where expressly permitted, dental students or other non-licensed attendees under specific registration categories. You represent that all information provided during registration is accurate, complete, and current. We reserve the right to deny or revoke access if eligibility requirements are not met.
3. ACCOUNT SETUP & SECURITY
Certain portions of the Website may require account registration. You are responsible for maintaining the confidentiality of your login credentials and for all activity under your account. Notify [email protected] immediately of any unauthorized use. We may restrict or suspend access at our discretion.
4. COURSE REGISTRATION
Registration is confirmed only upon receipt of full payment unless otherwise stated in writing. Course availability, instructors, content, schedules, and locations are subject to change. We reserve the right to cancel or reschedule a course if necessary.
5. PAYMENTS
Payments are processed through third-party payment processors. You authorize us to charge the payment method provided for all applicable fees. You are responsible for any applicable taxes.
6. CANCELLATIONS & REFUNDS
All course registrations are nonrefundable. In the event of an unforeseen circumstance, 4M Institute may, at its sole discretion, apply the registration amount as a credit toward a future course. No cash refunds will be issued.
7. USE OF THE PLATFORM; PROHIBITED CONDUCT
You agree not to
(a) share access credentials
(b) copy, record, distribute, or resell course content
(c) disrupt Website functionality
(d) engage in unlawful, misleading, or unsafe conduct
(e) violate any professional or ethical standards
We may suspend or terminate access for violations without refund or credit.
8. COURSE PARTICIPATION
Participation varies by registration type.
Licensed dentists may be eligible for hands-on participation and CE credits as specified.
Dental students and non-licensed attendees may attend lectures and, where expressly stated, participate in typodont-based workshops only. Live patient care or clinical procedures are prohibited for non-licensed participants.
9. NO PATIENT CARE / NO PHI
Do not upload, store, or transmit Protected Health Information (PHI) through the Website or Institute platforms. The Institute does not act as a HIPAA Business Associate.
10. INTELLECTUAL PROPERTY
All Website content and course materials are owned by 4M Institute or its licensors and are protected by law. You are granted a limited, non-exclusive, non-transferable license for personal educational use only. Recording, reproduction, distribution, or resale without written consent is prohibited.
11. PRIVACY
Your use of the Website and participation in Institute programs is governed by our Privacy Policy. By using the Website, you consent to our data practices.
12. ELECTRONIC COMMUNICATIONS & E-SIGN CONSENT
You consent to receive communications electronically. Electronic agreements, notices, and records satisfy legal requirements for written communications and signatures.
13. SERVICE AVAILABILITY; MAINTENANCE
We may modify, suspend, or discontinue the Website or any feature at any time, including for maintenance or updates, without liability.
14. LIMITATION OF LIABILITY; DISCLAIMER; INDEMNITY
To the maximum extent permitted by law, 4M Institute disclaims all warranties and shall not be liable for indirect, incidental, consequential, or punitive damages. Our aggregate liability shall not exceed the amount paid for the applicable course. You agree to indemnify and hold harmless 4M Institute from claims arising out of your participation or violation of these Terms.
15. COMPLIANCE
You are responsible for complying with all applicable federal and state laws, licensing requirements, and professional regulations related to your participation in Institute programs.
16. GOVERNING LAW; VENUE; SEVERABILITY
These Terms are governed by the laws of the State of California. Venue lies exclusively in California courts. If any provision is unenforceable, the remainder remains in effect.
17. FORCE MAJEURE
We are not liable for delays or failures caused by events beyond our reasonable control, including acts of God, labor disputes, utility failures, or government actions.
18. CHANGES TO THESE TERMS
We may update these Terms from time to time. The “Updated” date reflects the most recent version. Continued use after changes constitutes acceptance.
